Did it appear to fall apart as it went by? What did that look like?
  yes, blew up and made a massive cloud of debris, looked like a expanding
shockwave, as if from a bullet fired from a gun


Did it leave a persistent streak in the sky after it was gone?
How long did that last?
  yes, 15 minutes+, probably due to the sun shining on it below the horizon


How fast did it move? Use a 1 to 5 scale, 1 being
VERY slow, and 5 being extremely quick.

it
while we were on the road, after dark.  we pulled over and took some 
pictures
and marveled.
