Your form looks adequate for the most part, but you might want to consider
removing the entry for "radiant diameter". It essentially has no meaning for a
visual observer and may cause confusion as to where one can find this
information? Nearly all radiant diameters seem to be less than 2 degrees with
several being less than 1 degree. Whether the actual diameters be 2 degrees or
1 degree, it wouldn't matter if that information is listed on the form or not.
It wouldn't matter for one doing the Counting Method, nor for someone doing
the Plotting method.
