There are several forms in use for recording meteors already. Many observers
use a tape recorder while observing to reduce their "dead time" which is a
measure of how much time it takes to record the meteor information on paper.
But if you would still like to use a form, there are several at the home
page of the North American Meteor Network (NAMN). The address is:

Hi,

I though of an idea- why not have a form, which makes
it for you easier to fill in the records quicker,
loosing less observing time? Is there any need for
such a thing?
I'm still trying to find out the simplest way for
recording meteors, but on paper. hehehe...
