I use a custom made form I made myself. I guess it doesn't make much difference in what you are using, as long as it sticks to the rules (right fields, data...). The only thing that should be standardised is the report that is sent to IMO (meteor summary report).

It sure would be nice and less confusing if the 3 different Meteor
Observing
Orgs. used the same form. Whats wrong with that idea???
