As far as I know (I've only had to use the Moon for magnitude comparison a
couple of times) it is the total magnitude. Surface brightness is given in
magnitude per unit of area.
So yes, you would use the intensity of ground illumination to judge the
magnitude.

I'll let the experts on this list give some better and more detailed
answers.
