Fireball Report Form

If you have never before witnessed a fireball, or don't know what to report, use this series of questions to complete your report. Questions in BOLD are information we must have to process your sighting. With enough information, we will be able to add your observation to the fireball database.

  1. Your Name?

  2. Your Town/State/Country?

  3. An email address where we can reach you?

  4. Date and Time?

  5. What compass direction did the fireball appear from?

  6. What compass direction did the fireball DISAPPEAR from?

  7. How long, in seconds, were you able to see it in the sky?

  8. How many degrees off the horizon was it when it APPEARED? (As a reference, a closed fist held at arm's length is approximately 10 degrees.)

  9. How many degrees off the horizon was it when it DISAPPEARED?

  10. How bright did it appear? Like a star, Venus, the Moon, or the setting Sun?

  11. Did it have any color(s)?

  12. Did it appear to fall apart as it went by? What did that look like?

  13. Did it leave a persistent streak in the sky after it was gone? How long did that last?

  14. How fast did it move? Use a 1 to 5 scale, 1 being VERY slow, and 5 being extremely quick.

  15. Did you hear a sound? If yes, what was the time delay from sighting to sound?

  16. What is the closest Town/State to where you saw the fireball?
